Bring in any medical history you have of your pet when you come for your appointment.  It's best to keep a journal of your pet's health throughout his or her life, including behavioral shifts.  If you have something like this, share it with our team.  If not, let us know everything that you think will be important.  We can still work with whatever information you have. If you have never been to our clinic, please fill out the new client registration form prior to your appointment here. If you are not able to fill out the registration form prior to your appointment, please arrive 10 minutes early so that you will have time to fill it out before the start of your appointment time.

If you are coming in for a new puppy or kitten visit, please also bring in a fecal sample so that we can screen your new pet for worms. If you are coming in for a wellness visit for your bird, rabbit or other small mammal please bring in a detailed description of your pet's husbandry including pictures of cages or enclosures, detailed dietary information and any previous medical history or testing you have available. If you are coming in for a wellness visit for your reptile please also bring in information about any lighting, heat or humidity support equipment that you use for your pet.

For your first visit, please plan for your appointment to last anywhere from 30-60 minutes so that there is ample time for us to get acquainted with you and your pet, discuss their individual healthcare needs and answer any questions you may have for us in regard to your pet's healthcare going forward.
